Signups close July 22, 2026. League play starts the week of July 27, 2026.
League Schedule: Leagues run every day except Saturdays, from 5:30 PM – 10:30 PM. Sundays run during the day with the last game starting as late as 9:30 PM, depending on league size. Teams are accepted on a first-come, first-served basis according to the date and time of registration.

Youth league selected. Middle School and High School leagues include minor participants. Adult Participant Waiver

The team captain is signing on behalf of themselves as an adult participant. After signup, you'll receive a Team Waiver Signing Link to share with your roster — every adult teammate signs this same waiver online through that link before their first match. (Players may also sign in person at check-in if they prefer.)

Adult Participant Agreement, Release of Liability & Assumption of Risk

PLEASE READ CAREFULLY. THIS IS A LEGAL DOCUMENT THAT AFFECTS YOUR LEGAL RIGHTS.

In consideration of being permitted to participate in the sand volleyball league operated by Revolutions Lanes & Fun Center (the "League"), I agree as follows:

Age and Capacity. I certify that I am at least 18 years of age and legally competent to sign this Agreement.
Assumption of Risk. I understand that sand volleyball is a physical, contact-capable recreational activity that involves inherent and other risks of injury, permanent disability, or death, including but not limited to:
- collisions with other players, teammates, poles, nets, boundaries, or spectators;
- diving, sliding, jumping, or falling on sand or other surfaces, causing sprains, strains, fractures, dislocations, or head/neck injury;
- contact with the ball at high speed, including impact to the face, eyes, or head;
- sun exposure, heat exhaustion, heat stroke, dehydration, or sunburn;
- uneven playing surfaces, hidden objects in the sand, weather conditions, insect bites, or allergic reactions;
- exposure to communicable diseases, including viral or bacterial illness;
- travel to and from the facility.
I acknowledge that these risks may result from my own actions, the actions of others, or the actions or inactions of the Released Parties (defined below), including their negligence. I VOLUNTARILY AND KNOWINGLY ASSUME ALL SUCH RISKS.
Release, Waiver, and Discharge of Liability. I hereby RELEASE, WAIVE, DISCHARGE, AND COVENANT NOT TO SUE Revolutions Lanes & Fun Center, its owners, officers, directors, employees, agents, contractors, volunteers, officials, referees, sponsors, facility owners, landlords, and affiliates (collectively, the "Released Parties") from any and all claims, demands, actions, causes of action, damages, costs, and expenses of any kind, whether in law or equity, that I may have or that may accrue to me, my heirs, next of kin, spouse, executors, administrators, and assigns, arising out of or related to my participation in League activities, INCLUDING WITHOUT LIMITATION ANY AND ALL CLAIMS ARISING FROM THE NEGLIGENCE OF THE RELEASED PARTIES, whether such negligence is active or passive. This release does not extend to gross negligence, intentional acts, or claims that cannot be waived by law.
Indemnification. I agree to INDEMNIFY, DEFEND, AND HOLD HARMLESS the Released Parties from any loss, liability, damage, claim, cost, or expense (including attorneys' fees) they may incur due to my participation, including any claim brought by a member of my family or any third party arising from my participation.
Medical Treatment Authorization. I authorize the Released Parties to obtain emergency medical treatment for me if I am unable to authorize it myself. I am financially responsible for the cost of such treatment. I represent that I am in good physical condition and have no medical condition that would prevent safe participation.
Photo and Video Release. I grant the Released Parties permission to photograph and/or record me during League activities and to use those images or recordings for League promotional purposes without compensation, unless I have indicated a Photo/Video Opt-Out on this form.
Rules, Conduct, and Refunds. I agree to abide by League rules and the reasonable directions of League officials. The League may reschedule or cancel games due to weather, unsafe conditions, or other circumstances. Unsportsmanlike conduct may result in warnings, forfeiture, suspension, or removal from the League without refund. Team registration fees are non-refundable after July 22, 2026 (the signup deadline).
Governing Law, Venue, and Severability. This Agreement is governed by the laws of the State of Missouri. Any dispute shall be brought exclusively in the state or federal courts located in Buchanan County, Missouri, and I consent to personal jurisdiction there. If any provision is held invalid or unenforceable, the remaining provisions shall remain in full force and effect.
Entire Agreement. This Agreement contains the entire agreement between me and the Released Parties regarding my participation. I have not relied on any oral or written statements not contained in this Agreement.

ACKNOWLEDGMENT. I HAVE READ THIS AGREEMENT, UNDERSTAND IT, AND SIGN IT VOLUNTARILY. I UNDERSTAND THAT BY SIGNING IT I AM GIVING UP SUBSTANTIAL LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE THE RELEASED PARTIES FOR THEIR NEGLIGENCE.

I have received and read the League Rules & Facility Policies, and I agree that my team will follow them. I understand it is my responsibility as captain to share these rules with every player on my roster.

I have read and agree to the Adult Participant Agreement, Release of Liability, and Assumption of Risk above.

Photo/Video Opt-Out: I do not want my image (or my minor child's image) used by the League for promotional purposes such as social media, website galleries, or advertising. I understand I may still appear incidentally in photos or videos of League play; this opt-out is about promotional use only.

Parent/Guardian Waiver for Minor Participants

Because you selected a youth league (Middle School or High School), the parent or legal guardian of each minor player is required to sign this waiver. If you are the parent of a minor on the roster, you may check the box below to accept on their behalf now. The League will follow up to collect parent/guardian signatures for any other minor players before the first match.

Parent/Guardian Waiver, Release & Medical Authorization for Minor Participant

PLEASE READ CAREFULLY. THIS IS A LEGAL DOCUMENT THAT AFFECTS YOUR LEGAL RIGHTS AND THE RIGHTS OF YOUR CHILD.

In consideration of my minor child ("Minor") being permitted to participate in the sand volleyball league operated by Revolutions Lanes & Fun Center (the "League"), I, as parent or legal guardian, agree as follows:

Authority. I represent that I am the parent or legal guardian of the Minor identified in this Agreement and have full legal authority to sign this Agreement on the Minor's behalf and on my own behalf.
Assumption of Risk on Behalf of Minor. I understand that sand volleyball is a physical, contact-capable recreational activity that involves inherent and other risks of injury, permanent disability, or death, including but not limited to:
- collisions with other players, teammates, poles, nets, boundaries, or spectators;

On behalf of the Minor and myself, I acknowledge that these risks may result from the Minor's own actions, the actions of others, or the actions or inactions of the Released Parties (defined below), including their negligence. To the fullest extent permitted by law, I VOLUNTARILY AND KNOWINGLY ACCEPT AND ASSUME ALL SUCH RISKS on behalf of myself and the Minor.
Release of My Own Claims. I, on my own behalf and on behalf of my heirs, assigns, and personal representatives, hereby RELEASE, WAIVE, DISCHARGE, AND COVENANT NOT TO SUE Revolutions Lanes & Fun Center, its owners, officers, directors, employees, agents, contractors, volunteers, officials, referees, sponsors, facility owners, landlords, and affiliates (collectively, the "Released Parties") from any and all claims that I may have arising out of the Minor's participation, including without limitation my claims for medical expenses paid on the Minor's behalf, loss of the Minor's services or companionship, and any other derivative claims, INCLUDING WITHOUT LIMITATION ANY CLAIMS ARISING FROM THE NEGLIGENCE OF THE RELEASED PARTIES.
Release on Behalf of Minor. To the fullest extent permitted by law, I also RELEASE, WAIVE, and DISCHARGE any claims the Minor may have against the Released Parties arising out of the Minor's participation, including claims arising from the Released Parties' negligence. I acknowledge that Missouri law may limit the ability of a parent to waive certain claims on a minor's behalf, and I do not intend by this Agreement to waive any right the Minor has by law that cannot be waived by a parent.
Indemnification. I agree to INDEMNIFY, DEFEND, AND HOLD HARMLESS the Released Parties from any loss, liability, damage, claim, cost, or expense (including attorneys' fees) they may incur due to the Minor's participation, to the fullest extent permitted by law.
Medical Authorization. I authorize the Released Parties and any League official to (a) obtain emergency medical treatment for the Minor if I cannot be reached, and (b) authorize any medical treatment recommended by qualified emergency personnel. I am financially responsible for the cost of such treatment. I represent that the Minor is in good physical condition and able to safely participate. Relevant medical conditions, allergies, and medications are disclosed on this form.
Photo and Video Release. I grant permission for the Released Parties to photograph and record the Minor during League activities and use those images or recordings for League promotional purposes without compensation, unless I have indicated a Photo/Video Opt-Out on this form for that Minor.
Rules and Conduct. I agree the Minor will abide by League rules and I will support League officials in enforcing them. The League may reschedule or cancel games. Unsportsmanlike conduct may result in suspension or removal without refund.
Governing Law, Venue, and Severability. This Agreement is governed by the laws of the State of Missouri. Any dispute shall be brought exclusively in the state or federal courts located in Buchanan County, Missouri. If any provision is held invalid, the remaining provisions shall remain in force.

I am the parent or legal guardian of at least one minor player on this roster and I agree to the Parent/Guardian Waiver above on that Minor's behalf and my own behalf. I acknowledge that a separate waiver is required for any other minor on the roster and I will assist the League in collecting those signatures before the first match.
